Instructions: Print 2 copies of this form ! Keep 1 copy, it is your admission ticket and course confirmation. (You may need to submitt this to your department for reimbursment). Mail the other copy, with a check for the correct amount to: SMEMS, 31 Smith Place, Cambridge MA 02138.  All payments must be received by the end of the first day of class.  You will not be permitted to attend multi-day programs without payment being made.  Please do not ask for exceptions to this rule.  Under NO circumastnces will a candidate's name be submitted to OEMS as having completed a course without payment being made.  Acknowledgment of Video and Audio Tape Recording: Extremely rigorous verification and make-up procedures are in effect for all SMEMS courses.  Periodic video and photographic documentation WILL be used to document participant's attendance in class.  Please clear your schedule for the entire duration of the course, allowing appropriate time to commute to and from work or home.  Any time that is missed, previously expected or not, must be made up in the week following the completion of the course.  Course rosters will be submitted to OEMS showing "Incomplete" if make-up has not been made within 7 business days of course completion.  Also, please note MONITORED make-up times will be posted prior to the end of class.  Again, no exceptions to this will be made.  Start Time:  MEDIC Refreshers begin each day at 9:00 am. Late arrivals must complete make-up work.  Class ends at 4:00.  ACLS Renewal Times are 8am to 5pm. Please make arrangements to accomodate this.
